Submitting Testing and
Medical Records
Tests and records are key to ensuring you’re safe to begin treatment and direct your care. Be sure your records are ready to go with these handy tips:
My Previous Test Results:
Are they expired?
Most tests must have been performed in the United States within one year to be valid; tests from abroad may help inform treatment, but are generally insufficient
- ABO and Rh Type (blood type)
- Genetic Carrier Screening
- Karyotype Chromosomal Analysis
- Pap Smear:
- If there is no prior history of an abnormal result
- Pap Smears with abnormal results will require discussion with clinicians
- Negative HPV results may be valid for 3-5 years, depending on individual circumstances; our clinicians can advise
- All other tests:
- Required Blood Panel
- Hepatitis B & C
- HIV, HTLV
- Chlamydia & Gonorrhea
- Many others
- SIS / HSG
- Antibody Screen
- Sperm Count or Semen Analysis
Ensuring My Records are Valid for Use
In addition to being within a valid time window, any test results or other medical records submitted to Life IVF Center must contain all five of these criteria to be legally acceptable
Name on all records must match with government ID or previous name with proper legal name change documentation
Date of Birth listed on any medical records must match with the date of birth on your government ID
Reference range showing target test levels for the population are a mandated requirement for test results
The exact date of the sample collection for the test must be included on all result documentation
The name, address / location, and contact information of the testing facility must be clearly listed
