1. Scope of This Policy

This Privacy Policy describes how Frank D. Yelian MD PC DBA Life IVF Center (“Life IVF,” “we,” “us”) collects, uses, and protects information you provide when using the Life IVF Patient Learning Hub (“Hub”).

This policy applies solely to the Hub. It does not govern information collected through your clinical care, patient portal, or other Life IVF systems.

2. Information We Collect

When you register for the Hub, we collect:

  • First name
  • Last name
  • Email address
  • Password (stored in encrypted form; never visible to staff)

When you use the Hub, we automatically collect:

  • Course and lesson completion records (which modules you have marked as reviewed and when)
  • Login activity (date and time of access)
  • Standard technical data collected by our website platform (IP address, browser type, device type)

We do not collect: date of birth, phone number, health insurance information, diagnosis, treatment details, or any information from your clinical record.

3. How We Use Your Information

We use your information to:

  • Create and maintain your Hub account
  • Track your progress through educational courses
  • Allow Life IVF staff to reference your completion records when discussing educational materials with you
  • Send you administrative emails related to your Hub account (e.g., password reset)
  • Improve Hub content and functionality

We do not use your Hub data to make clinical decisions, and it is not incorporated into your medical record.

4. How We Do Not Use Your Information

Life IVF will not:

  • Sell, rent, or share your Hub account information with third parties for marketing purposes
  • Link your Hub account to your clinical or insurance records
  • Use your completion records as a substitute for formal informed consent
  • Use your Hub data to determine your eligibility for treatment
  • Send you marketing communications based solely on your Hub activity without your separate consent

5. Data Storage and Security

Your Hub account data is stored on secure servers provided by our learning management platform (LearnDash/WordPress). We maintain reasonable administrative, technical, and physical safeguards appropriate to the sensitivity of the data collected.

No internet transmission is completely secure. While we work to protect your information, we cannot guarantee absolute security.

6. HIPAA Notice

The Life IVF Patient Learning Hub is an independent educational platform. The information collected through the Hub — your name, email address, and course completion records — is not maintained as part of a designated record set under HIPAA and is not subject to HIPAA’s Privacy or Security Rules in this context.

Your separately maintained clinical records at Life IVF are governed by our HIPAA Notice of Privacy Practices, which is provided to you as part of your clinical intake. If you have questions about your clinical records, please contact our office directly.

7. Email Communications

Life IVF may send you emails related to your Hub account, such as enrollment confirmations, course reminders, or password reset instructions. These emails will originate from a Life IVF domain and will reference the Learning Hub.

We will not send emails that disclose the nature of your fertility treatment or your status as a patient in the subject line or preview text.

You may opt out of non-essential Hub communications at any time by contacting us at the address below.

8. Data Retention

We retain your Hub account information for as long as your account is active. If you request deletion of your account, we will remove your personal information within 30 days, except where retention is required for legitimate administrative or legal purposes.

Course completion records that have been referenced in a patient communication may be retained in administrative records for up to seven years consistent with California healthcare recordkeeping standards.

9. Your Rights (California Residents)

Under the California Consumer Privacy Act (CCPA), you have the right to:

  • Know what personal information we have collected about you through the Hub
  • Request deletion of your Hub account information
  • Opt out of the sale of your personal information (we do not sell personal information)

10. Children

The Hub is not directed at individuals under 18 years of age. We do not knowingly collect information from minors.

11. Changes to This Policy

We may update this Privacy Policy periodically. The date of the most recent update appears at the top of this document. Continued use of the Hub after an update constitutes acceptance of the revised Policy.
