2. Who May Use This Platform

Access to the Hub is available to current and prospective patients of Life IVF and their authorized partners or representatives. By creating an account and using the Hub, you confirm that you are at least 18 years of age and are using the platform in connection with your own fertility care or that of a person for whom you are legally authorized to act.

3. Account Registration

To access course content, you will create an account using your first name, last name, and email address. You are responsible for keeping your login credentials confidential. Life IVF is not responsible for unauthorized access to your account resulting from your failure to protect your credentials.

Your account information is used solely to provide and administer your access to the Hub. It is not linked to your clinical record at Life IVF, is not shared with your insurance carrier, and is not used in clinical decision-making.

4. Nature of Content and Completion Records

The Hub includes courses, lessons, and modules covering topics such as treatment procedures, financial policies, billing practices, and clinic expectations. Some modules include a “Mark as Reviewed” function that allows you to indicate that you have accessed and reviewed the material.

Important:

• Marking a module as reviewed indicates that the material was made available to you and that you accessed it. It does not constitute informed consent, a legally binding acknowledgment, or a waiver of any rights.
• Completion records are maintained for administrative reference only and are not part of your medical record.
• Use of this platform does not replace discussion with your physician or care team. If you have questions about your treatment, please contact Life IVF directly.
• Life IVF staff may reference your completion records when discussing educational materials with you, for example to confirm that a resource was provided or to offer clarification. This does not transfer legal or financial responsibility for matters that require separate formal acknowledgment.
• You agree not to rely on Hub content as a substitute for direct medical, financial, or administrative guidance from Life IVF staff.

5. No Medical Advice

Content on the Hub is provided for general educational purposes. It describes typical procedures, common policies, and general information about fertility treatment. It does not account for your individual medical history, diagnosis, or circumstances. Any reliance on Hub content is at your own risk. Do not make medical decisions based solely on Hub content. Always consult your physician.

6. Financial Information

Some Hub content describes Life IVF’s financial policies, fee structures, and billing practices. This content is provided for educational reference only and does not constitute a binding quote or agreement. Actual charges, package terms, and payment obligations are governed exclusively by the financial agreements you sign with Life IVF. In the event of any conflict between Hub content and a signed financial agreement, the signed agreement controls in all cases.

7. Intellectual Property

All content on the Hub, including text, slide presentations, graphics, and course structure, is the property of Frank D. Yelian MD PC or its content licensors. You may access and use content for your personal educational purposes only. You may not reproduce, distribute, or share Hub content without written permission from Life IVF.

8. Disclaimer of Warranties

The Hub is provided “as is” and “as available.” Life IVF makes no warranties, express or implied, including but not limited to accuracy, completeness, reliability, or fitness for a particular purpose. Life IVF does not guarantee that the platform will be uninterrupted, error-free, secure, or free of harmful components. Educational content may not reflect the most current clinical or administrative practices at all times.

9. Limitation of Liability

To the fullest extent permitted by California law, Life IVF shall not be liable for any indirect, incidental, consequential, special, or punitive damages arising from or related to your use of, or inability to use, the Hub or its content.

Life IVF’s total cumulative liability, if any, shall not exceed $100.

This limitation applies regardless of the legal theory asserted and even if Life IVF has been advised of the possibility of such damages.

This limitation does not apply to liability that cannot be limited under applicable law, including claims arising from Life IVF’s gross negligence or willful misconduct in the provision of clinical care.

10. Dispute Resolution

If you have a concern about the Hub or these Terms, please contact Life IVF in writing at the address below. We will make a good-faith effort to resolve your concern informally within 30 days. If informal resolution is unsuccessful, disputes shall be litigated exclusively in the Superior Court of California, County of Orange or Los Angeles, at Life IVF’s election. You and Life IVF each waive the right to participate in any class, collective, or representative action related to this platform.

11. Changes to These Terms

Life IVF may update these Terms from time to time. Continued use of the Hub after any update constitutes acceptance of the revised Terms. The date of the most recent revision appears at the top of this document.

12. Governing Law

These Terms are governed by the laws of the State of California without regard to conflict of law principles.

13. Acceptable Use and Access Termination

You agree to use the Hub only for its intended educational purpose. You may not misuse the platform, attempt to gain unauthorized access, interfere with its operation, or use it in a manner that could harm Life IVF or other users.

Life IVF reserves the right to suspend or terminate access to the Hub at any time, with or without notice, for any reason, including misuse of the platform.
